Identification (ID) card forms

​Application for ​a​ Driver's License or Identification Card​ (PS33100)	Apply for a Minnesota instruction permit, driver's license or identification card.
​​Application for a Driver's License Fee Refund​ (PS31081)	Request a refund of identification card fees you paid.
Autism spectrum - mental health - emergency/caretaker contacts (PS34300)	Add or remove emergency contact or caretaker information from your driver record. Add or remove the Autism Spectrum and/or Mental Health Indicator from your card.
Victim of identity theft (PS31300)	Add a law enforcement alert of identity theft to your record.
Eligibility verification of reduced fee ID card (PS33117)	Provide DVS proof of eligibility for a reduced fee ID card. This form must be completed by a licensed medical professional or case manager.
Homeless youth ID (PS2908)	Provide DVS proof of eligibility for a homeless youth ID card. This form must be signed by a Human Services agency employee, school staff, or school social worker.
Status check information	Learn more about status check information.
Certificate of translation form (PS33210)	Provide proof of document translation. This form must be completed in English.
Customer service recognition or complaint	Submit feedback to DVS about a customer service experience.
