Driver compliance requests and forms
Depending on your situation, you may be required to complete and submit documentation to Driver and Vehicle Services (DVS).
Make requests online
You can request a:
- Refund, select More Driver Services, then Request a Driver's License Refund.
- Alcohol restriction be removed from your record
- Administrative review
- Seven-day temporary license
Submit documents online
You can submit:
- Documentation to satisfy a civil judgement
- Auto insurance information
- A special review awareness
- Documentation to satisfy Vanessa’s Law requirements
- Proof of fines paid outside Minnesota
- A non-residency statement
- A death notice

| Request for examination of driver (PS31924) | Request reexamination of someone's driving ability. |
| Restriction removal (PS2122) | Request alcohol restriction on your license be removed. |
| Special review awareness (PS31086) | Acknowledge you have multiple alcohol or controlled substance offenses on your driving record. |
| Vision report (PS30338) | Submit a vision report. Must be signed by licensed vision examiner. |
| Voluntary surrender of driving privileges (PS33061A) | Request your driving privileges be voluntarily surrendered. |
| Withdrawal of parental consent (PS33061B) | Withdraw/reinstate parental consent of the driving privileges of a minor child. |
