Print and complete the form
- Print and complete the Petition for Variance to DVS Policy 125-1000 form.
- If you are requesting a variance to complete a record view for a motor vehicle record, include the vehicle identification number (VIN) or license plate number.
- If you are requesting a variance to complete a record view for a driver license record, include the driver’s license number.
- Each variance request must be submitted separately.

- Please retain a copy of the final approved form in your office files for three years.
