Petition for variance to DVS Policy 125-1000

Complete the following steps to file a Petition for Variance to DVS Policy 125-1000 if you work for an organization:

  • Where you are the sole individual with access to the Driver and Vehicle Services (DVS) information system – MNDRIVE or e-Services for Business – and you need to view a record that would violate Policy 125-1000.

  • That employs multiple people and viewing a specific record would violate DVS Policy 125-1000 for all users (such as all family members).

You must receive written approval from DVS before viewing the record.

Print and complete the form

  • Print and complete the Petition for Variance to DVS Policy 125-1000 form.
    • If you are requesting a variance to complete a record view for a motor vehicle record, include the vehicle identification number (VIN) or license plate number.
    • If you are requesting a variance to complete a record view for a driver license record, include the driver’s license number.
  • Each variance request must be submitted separately.

Submit the form to DVS

DVS will review the form

  • DVS will review the submission and respond back within five business days with a written approval or denial. 
    • If the request is approved, you will receive instructions on when you must have the record viewed by.
    • If the request is denied, you will receive further instructions on why the request was denied.

Approval process

  • If approved and once the record view is complete, you are required to email the DVS auditor and include the following:
    • Confirmation number (if applicable).
    • Transaction or session ID (if applicable).
    • Date and time of record view.

Documents you will receive from DVS

  • DVS will return the form with the "Office Use Only" section completed. 
  • Please retain a copy of the final approved form in your office files for three years.
