Driving record request procedure
Driver’s license record requests
You can request copies of your own driving record by completing a Record Request Form (PS2502).
If you are requesting someone else’s record, you must select one of the acceptable reasons given on the form to receive the information. Alternatively, you can provide written authorization from the person whose record is needed by submitting a Grant Access to Record Authorization Form (PS2506).
Driver and Vehicle Services (DVS) reviews each request to determine whether the requester is allowed to receive the information.
You can obtain a copy of your driving record right away at any exam station (non-certified only), from a full-service deputy registrar office, or through your MyDVS account. You can also mail the Record Request form with a check or money order for the fee to DVS.
There is a fee for obtaining a record.
Crash record requests
To request copies of law enforcement reports on crashes in Minnesota, complete the Crash Record Request (PS2503) form.
You can obtain a report in person at DVS, by mail at DVS, or from a full-service deputy registrar office (individuals involved in the crash only).
