Ignition Interlock Device Program application

To participate in the program, you must: 

  • Be at least 18 years of age
  • Own or drive a vehicle with valid insurance
  • Have a Class D type vehicle (no commercial vehicles)
  • Go to an office that offers license plate services to purchase special registration plates if your license plates have been impounded. Once you are successfully enrolled in the program, you can return to get regular license plates
  • Not have outstanding withdrawals
  • Not be subject to Vanessa's Law if your driving privilege was revoked before age 18

Application steps

Apply for a new driver's license

Apply for a new Class D driver’s license and pay the application fee. 

  • Commercial driver's license (CDL) privileges are not allowed while participating in the program.  Appropriate testing and application for CDL privileges can only be renewed upon completion of the program
  • Use the Where’s my license? feature to see when your restricted license has been printed

Provide proof of vehicle insurance

Everyone on IIDP must have insurance and provide proof of insurance when applying.

The insurance identification card must contain:

  • Your Minnesota driver’s license number
  • Your full name and date of birth
  • Name of insurance company
  • Policy number
  • Vehicle identification number (VIN)
    • Must match VIN of the vehicle with the device  
  • Effective date of policy
    • Policy cannot be expired

Certificate of insurance

A certificate of insurance will be required if your driving record shows a history of an insurance-related incident. The certificate of insurance must be in your name.

Complete additional paperwork

Revoked drivers

Sign and notarize the Special Review Awareness form, if required. 

Canceled and denied as inimical to public safety (Canceled-IPS) drivers

Sign and date the Last Use Statement

Submit your paperwork to DVS

Get an ignition interlock device installed

Select an ignition interlock manufacturer and have the device installed. 

You may have the manufacturer install the device at any time, but you must wait until your driving privilege status is valid before driving the vehicle.

Wait until your license status is valid

We will review your application and we may need additional information. 

Check your driving privilege status while you wait. If it is listed as valid, you are legal to drive with an ignition interlock device installed in your vehicle. You will also receive a conditional reinstatement letter when you are legal to drive.

Understand the fees you owe

You do not need to pay your reinstatement fees before you apply for the program.  Use the driving privilege lookup to learn more about the fees you owe.

Have your provider complete a substance use disorder assessment*

Your provider must complete a substance use disorder assessment and you must enroll in a treatment or other program if needed. 

The treatment center, the assessor, or the alcohol or drug counselor must fax the assessment and proof of enrollment directly to Driver and Vehicle Services at 651-797-1738. Ask the provider to include your driver’s license number or date of birth on the assessment or enrollment for verification purposes. 

*This step does not apply to first-time offenders, unless you were convicted of criminal vehicular homicide.
